PORT KLANG: A senior citizen died after he fell into a lift shaft at Klang Villa Apartments on Monday.

Mohd Din Salahuddin, 64, passed away about 15 minutes after the fall due to severe head and body injuries in the incident which took place at about 4pm.

The incident occurred when Din had walked into the lift shaft without realising that the lift was stuck on an upper floor.

Just moments earlier, the lift had momentarily stalled and its doors opened unexpectedly on the fourth floor.

Some five residents who were in the lift exited the lift and took the stairs down.

Seconds later, Din walked into the lift shaft when the doors opened again, unaware that the lift had gone up to the fifth floor.

Din’s son, Mohd Sobri, said he was alerted of the incident by a neighbour who came running to his apartment unit.

“My neighbour screamed ‘Sobri! Sobri! Your father fell down the lift shaft’. I immediately ran to the fourth floor and saw my father who was bloodied on the lowest floor. I took the stairs down but had to wait for a few minutes for the apartment’s Joint Management Body (JMB) to open the lift door. I was stopped from reaching out for my father as the other residents told me that the ambulance was on its way.

“After 15-minutes of waiting, the ambulance had yet to arrive and afraid that the lift would drop and crush my father who was still alive but unconscious at that time. However, five minutes after pulling him out, he passed away,” he said when met at the apartment earlier today.

SINGAPORE – While taking his son to school on Friday morning, businessman Ben Tng was shocked to see that the lift door remained partly open as the lift moved.

In a video that Mr Tng, 42, posted on Facebook, the lift at Block 114 Edgefield Plains descended more than 10 floors with its inner door partly open.

“I held on to my son tightly with one hand while I took the video with the other hand,” Mr Tng told The Straits Times.

He took the lift a second time later that morning with his younger son, who was going to kindergarten and the lift was still not fixed, he said.

When he returned home at 9am, he saw that the lift was still operating.

Mr Tng, wrote on Friday at 9.24am: “7.25am, I called the 24hrs hotline. I came home at 9.00am. A group of residents was crowding at the lift lobby, wanting to call the town council. I told them I called at 7.25am, and until now nobody came! Residents were worried about young children too.”

He made a second call to the Town Council at around 9am, he said. He took the same lift up, and filmed another video.

When he left his home again at 10am, the lift was under maintenance.

The lift was repaired when Mr Tng returned home at 3.20pm.

Yemeni girl seriously injured after legs trapped in lift doors of Kajang apartment

ALA LUMPUR: A 12-year-old Yemeni girl was seriously injured when both her legs were caught in the doors of a lift at an apartment in Taman Sepakat Indah 2, Sungai Chua, Kajang near here Friday.

Selangor Fire and Rescue assistant director Mohd Sani Harul said the incident occurred about 8.15pm on the 14th floor of the apartment as she was returning to her family unit.

He said firemen who arrived at the scene after being informed of the incident managed to open the lift doors and bring the girl out.

HYDERABAD, India – Indian police said Tuesday they have detained a school principal and a teacher after the death of a three-year-old girl whose head became stuck in the doors of an elevator.

The child, Syeda Xainab Fatima Jaffer, was taking the lift up to her classroom in the southern city of Hyderabad with her teachers when the horrific accident occurred.

A Ravinder, deputy commissioner of police, said preliminary investigations showed that “the girl died after her head got stuck in the gap when she along with her teachers got into the elevator”.

It is not clear what caused the accident.

“The principal, the school teacher who was said to be accompanying the kids in the elevator and a few others have been taken into custody,” said Ganga Reddy, a senior police officer.

Her distraught mother accused the school of negligence and demanded action be taken against its management.

Dog leash too thin for lift doors to detect.

Probes into how a woman’s hand was cut off by a lift has found her dog leash was too thin.

The lift doors’ protective devices were working on the day Madam Khoo Bee Wah’s left hand was severed.

But the dog leash that was looped around her wrist was only 2mm thick and was too thin to be detected.

If the lift doors were closing, they would reopen if an object was placed within the detectable range of the protective devices.

During the incident, however, the sudden dragging of the leash in a downward direction through the bottom of the lift cabin doors when the lift was ascending likely caused a gap.

Madam Khoo’s hand was pulled towards the gap because she was unable to react in time to release the leash.

The AE recommends that the public be more aware of precautions such as keeping small and thin objects, such as a dog leash or dangling straps of backpacks, away from lift doors.

SINGAPORE — Releasing findings made by an independent authorised examiner for the Tah Ching Road lift mishap — who found that the safety devices were working properly that day — the Building and Construction Authority (BCA) said it will enhance public awareness on the precautions to take when using lifts.

That morning, Mdm Khoo had entered the lift before her leashed dog. When the doors closed, the dog was still outside the lift with its leash straddled across the lift doorway.

The doors’ protective devices were designed to reopen or remain open if there is an obstruction in the doorway. However, they were not designed to detect objects less than 10mm thick. The dog leash was 2mm thick.

After the lift doors closed, the lift ascended, tightening the leash that was looped around Mdm Khoo’s wrist and pulling her down to the floor. On the ground floor, her dog was pulled upwards until it was caught at the top of the lift lobby door.

The pull could have caused the lift doors to open slightly, pulling Mdm Khoo’s hand through the small gap crushing and severing it between the doors and the inner parts of the lift lobby doors. The lift eventually stopped as the opening of the lift cabin doors triggered the emergency stop feature.

“All the door protective devices were tested and verified to be working according to their specifications. The lift doors were also observed to have responded to obstructions during the rescue operation by SCDF (Singapore Civil Defence Force),” said the BCA.

Public sector seminar participants trapped in Sarawak hotel lifts

It was sheer panic at a hotel in Kuching this morning when the lifts suddenly jammed, trapping scores of participants rushing to attend the Malaysia-Singapore Public Sector Leaders seminar.

But thanks to quick action by the Fire and Rescue team as well as hotel staff, everyone was out safely after half an hour of anguish.

The incident also caused the seminar session to be delayed and the five-star hotel’s management expressed their apologies to Tan Sri Dr Ali Hamsa, chief secretary to the government.

Taman Jurong grassroots leaders visited the elderly woman and her family members at NUH on Friday afternoon. She was conscious but needed further medical treatment to stablise her condition.

The Building and Construction Authority (BCA) has directed Jurong Town Council to investigate the incident.

“Jurong Town Council is required to appoint an Authorised Examiner (AE) to conduct an investigation to determine the cause of the incident and to submit the investigation report to BCA,” the authority said.

“The appointed AE is also required to propose repairs to the lift, if necessary, and engage a registered lift contractor to carry out the recommended repair works.”

The BCA is also requiring Jurong Town Council to immediately appoint an AE to inspect the other lift in the same block to confirm that it is safe to be used.

“The investigation and proposed measures must be carried out to the satisfaction of the Commissioner of Buildings before the incident lift B is allowed to operate,” the BCA added.

TODAY reports: An elderly woman left’s hand was severed by lift doors at a HDB block in Taman Jurong.

SINGAPORE: An elderly woman’s left hand was believed to have been severed by lift doors at a Housing and Development Board (HDB) block in Taman Jurong on Friday (Oct 9).

The Singapore Civil Defence Force (SCDF) said it was alerted to the incident at Block 322 Tah Ching Road at 7.27am on Friday.

When they arrived, they found the elderly woman stuck in the lift on the ground floor with her left hand trapped in between the lift doors.

The SCDF released the woman using rescue tools to pry open the lift doors. She was conveyed to National University Hospital (NUH) with a serious left hand injury. The hospital was alerted to be on standby.

TODAY understands that the woman’s left hand was retrieved by paramedics, packaged in ice and taken to NUH with her.
