Past SARS-CoV-2 infection protection against re-infection: a systematic review and meta-analysis – The Lancet

..

Findings: Our meta-analyses showed that protection from past infection and any symptomatic disease was high for ancestral, alpha, beta, and delta variants, but was substantially lower for the omicron BA.1 variant.

Interpretation: Protection from past infection against re-infection from pre-omicron variants was very high and remained high even after 40 weeks. Protection was substantially lower for the omicron BA.1 variant and declined more rapidly over time than protection against previous variants.

Findings

We identified a total of 65 studies from 19 different countries. Our meta-analyses showed that protection from past infection and any symptomatic disease was high for ancestral, alpha, beta, and delta variants, but was substantially lower for the omicron BA.1 variant. Pooled effectiveness against re-infection by the omicron BA.1 variant was 45·3% (95% uncertainty interval [UI] 17·3–76·1) and 44·0% (26·5–65·0) against omicron BA.1 symptomatic disease. Mean pooled effectiveness was greater than 78% against severe disease (hospitalisation and death) for all variants, including omicron BA.1. Protection from re-infection from ancestral, alpha, and delta variants declined over time but remained at 78·6% (49·8–93·6) at 40 weeks. Protection against re-infection by the omicron BA.1 variant declined more rapidly and was estimated at 36·1% (24·4–51·3) at 40 weeks. On the other hand, protection against severe disease remained high for all variants, with 90·2% (69·7–97·5) for ancestral, alpha, and delta variants, and 88·9% (84·7–90·9) for omicron BA.1 at 40 weeks.

Interpretation

Protection from past infection against re-infection from pre-omicron variants was very high and remained high even after 40 weeks. Protection was substantially lower for the omicron BA.1 variant and declined more rapidly over time than protection against previous variants. Protection from severe disease was high for all variants. The immunity conferred by past infection should be weighed alongside protection from vaccination when assessing future disease burden from COVID-19, providing guidance on when individuals should be vaccinated, and designing policies that mandate vaccination for workers or restrict access, on the basis of immune status, to settings where the risk of transmission is high, such as travel and high-occupancy indoor settings.

A man in Ende, East Nusa Tenggara, was recently arrested for the murder of his fiancée during a fit of rage. Local police said the anger was triggered by the victim’s accusations that he had sex with a cow.

Ende Police Crime Unit Chief Yance Kadiaman said that the 44-year-old suspect, identified by his initials MO, who lives in Jopu Village, Wolowaru, assaulted his betrothed, AR, 35, on two occasions.

The first assault occurred on Feb. 4, during which he attacked her with his bare hands. On the second occasion, on Feb. 7, MO reportedly hit AR with a wrench, which proved a fatal blow.

AR died due to brain damage the next day after being hospitalized at the Santo Antonius Jopu Hospital.

“The suspect was upset with the victim because she accused him of committing adultery, often asking for sex, and that he had sex with a cow on their farm,” he said on Monday.

Under Indonesian law, MO may face 15 years behind bars if proven guilty of murder.

Authorities have detained a suspect, identified as Mikhael Oi, for the death of a 35-year-old woman, identified as Augustina Rere, according to a report by Detik.com.

Fatal consequences

The victim was attacked on two separate occasions, where she sustained fatal injuries in the second assault, the police said.

The man first attacked the woman on 4 Feb when he hit her with his bare hands. The second assault happened on 7 Feb when hit her with a wrench, which resulted to a serious physical injury.

Ontario judge was wrong to rely on anti-vaccine misinformation: Court of Appeal

Instead of evaluating the evidence, a Hamilton judge “embarked on a lengthy discussion about whose materials were more thought-provoking, which has no bearing at all on whether the respondent’s materials were admissible.”

By Jacques Gallant Courts and Justice Reporter

Mon., Feb. 6, 2023

In granting a mother decision-making authority regarding her children getting vaccinated against COVID-19, a Hamilton judge erred in relying heavily on the anti-vaccine posts the woman submitted to the court — “nothing but something someone wrote and published on the internet,” Ontario’s top court ruled Friday.

The Court of Appeal overturned the decision made last year by Superior Court Justice Alex Pazaratz, and instead granted authority to the father.

The parents in the case are separated and are split on the issue of vaccination for the two youngest children, who live with their mother.

The father, identified only as C.G., argued in court last year that there’s no medical reason not to vaccinate the children. The mother, identified as J.N., argued there was sufficient doubt about the vaccine’s safety and effectiveness. The children said they did not want to be vaccinated.

The Court of Appeal’s decision is not an order that the children — who were 10 and 12 at the time of Pazaratz’s ruling — be vaccinated. It simply means that it is now solely the father’s responsibility to address the topic of COVID-19 vaccines with them.

“There is no reason to doubt the appellant’s motivation and stated desire to approach this very sensitive issue in a measured way and with a view to the children’s best interests,” wrote Justice Jonathon George for a unanimous three-judge appeal panel.

Reposting a thread by Frank Han MD on heart transplants and vaccine requirements

There is a great deal of consternation about heart transplants and vaccine requirements. This thread is meant to explain to the public why our partners in heart transplant are so incredibly strict.

Heart transplant is one of the most scarce resources that exist. Big reason? A donor has to be deceased for a heart to be available. Even then, a separate medical team has to be sure the heart is healthy enough survive two surgeries (leaving the donor, getting to destination).

If fully functional hearts could be delivered at some point in the far future on a 3d printer, this will no longer be an issue. That’s an active area of research but not a reality.

Heart transplants come with a heavy medical appointment demands and heart procedures on the receiving patient to make sure the transplanted heart is functioning properly and stays healthy. Deviation from our protocol puts lives in direct danger.

Heart transplants come with mandatory immunosuppression at a dose tailored to the patient. This is not negotiable; this also places patients at risk for infections, including really serious ones. This is why a catch up vaccine schedule is required prior to transplant.

Just picking and choosing your vaccine schedule puts the heart transplant candidates life at risk. Click bait headlines about pundits getting upset don’t speak to the medical reality of why vaccination is necessary.

So don’t get your heart transplant information from emotion grabbing headlines.
